Purpose

The Post Year 1 (PGY1) Pharmacy Residency at LifeLong Medical Care & Touro University California College of Pharmacy (TUC COP) aims to use the knowledge and skills learned during the Doctor of Pharmacy education to prepare advanced practice pharmacists in the primary care setting. By the end of this residency, the pharmacist will be able to practice as a highly qualified independent practitioner in a variety of primary care settings.

Program Overview

The PGY1 Pharmacy Residency offers 2 positions and is a 12 month postgraduate curriculum that offers an emphasis in ambulatory care by providing a longitudinal ambulatory care core experience at LifeLong Medical Care. In addition, the resident is offered (3) six week elective rotations at one of our affiliated Bay Area hospitals or clinics.

During their time at LifeLong Medical Care, the residents will work closely with Dr. Khosraviani to help manage patient’s chronic disease states such as Type 1 and Type 2 diabetes mellitus, hypertension, and hyperlipidemia. The residents will work under a collaborative practice agreement allowing pharmacists to order medications and labs in collaboration with physicians, nurses, and physician assistants. They will also be involved in point of care A1c and blood glucose testing, in home health visits with social workers, teaching health and wellness education for patients, medication reconciliation, and lifestyle modification counseling.

Each week, residents will spend 2-3 days rounding with preceptors and providers to treat patients, 2 days conducting teaching or volunteer activities, and 1 day working on research projects. Upon completion of this residency, the resident will be awarded an ASHP accredited residency certificate, in addition to a teaching certificate.

Rotation Schedule

The residents will complete a longitudinal 12 month ambulatory care core rotation at LifeLong Medical Care, in addition to (3) six week elective rotations and 4 weeks of dedicated project time.

• Electives: The residents will complete their electives at any of the TUC associated Bay Area Hospitals and Clinics. There is a wide array of learning experiences to choose from depending on the residents’ interests. Elective options include:
• Infectious Disease
• Critical Care
• Trauma
• Emergency Medicine
• Psychiatry (ambulatory care)
• HIV/Hepatitis
• Oncology (both ambulatory and acute care)
• Transitions of Care
• Academia
• Primary care at other TU affiliated clinic
• Anti-coagulation clinic

Teaching Certificate

The residents will participate in a longitudinal teaching certificate program offered through TUC COP. Residents will have the opportunity to deliver didactic lectures, precept small group discussions, participate in the pharmacy lab and student run free clinics, and develop a teaching portfolio. Upon completion of the program, the residents will receive a formal teaching certificate.

Research Training

TU COP offers a formal research training program for the TUC COP residents. Monthly seminars are conducted to facilitate the progress of the resident’s research project. By the end of the 12 months, residents are fully prepared for Western States and manuscript submission.

Salary and Benefits

The residents will be awarded similar benefits as the TUC COP practicing faculty, with access to library, statistical software, and laptop privileges.

• Salary: roughly $48,000 annually paid twice monthly
• Insurance: Medical/Dental/Vision plan offered
• Vacation/Sick Leave: two weeks paid vacation with sick leave
• Professional leave: to be used for conference attendance and/or job/PGY2 residency interviews

Travel Stipend

The residents are provided with a stipend to support travel, lodging, and registration fees for the purposes of professional conferences. The residents are expected to attend either CSHP Annual Seminar or ASHP Midyear Clinical Meeting, in addition to Western States Conference at the end of the residency year.

OLE Health is a Federally Qualified Health Center in Napa, California and the primary site for the residency training. At OLE Health, majority of our patients are low-income, uninsured and Spanish speaking workers that have not historically received high quality of care at other institutions. To optimize care, pharmacist are truly integrated into the patient management.

Purpose

Purpose

Learning Experiences

The PGY1 Residency Program is a one-year postgraduate program that provides training and experience in the provision of pharmaceutical care and quality medication education to a diverse patient population in various areas of pharmacy practice.

The goals of the program are to foster development of leadership, professional education, and comprehensive clinical practice skills such that residents will become effective and marketable pharmaceutical care providers in a variety of practice settings. NorthBay Healthcare is a private, not for profit, independent healthcare system in Solano County consisting of two community hospitals, multiple primary and specialty care clinics and a Cancer Center.
