Kristen Herzik, PharmD, BCPS
Biography
Dr. Herzik is an assistant professor of clinical sciences at Touro University California College of Pharmacy. She obtained her Doctor of Pharmacy degree from Touro University California and completed her PGY1 training at Touro University in conjunction with San Francisco General Hospital. Dr. Herzik has practiced as a Transitions of Care pharmacist for over 6 years and currently at Sharp Grossmont Hospital in La Mesa, CA where she works mostly with patients that have heart failure. She has several publications related to transitions of care pharmacy and currently serves as Co-Chair of the Practice Committee of APhA’s Transitions of Care Special Interest Group.
